Abstract: At 6:30 a.m. on April 4, 2015, an employee reported to his employer's main office and picked up Freightliner Truck with double bottom dump trailer attached. After leaving for the loading site at about 6:30 a.m., the employee arrived at the plant where he was expected to load at 7:20 a.m. He entered the plant, turned around, left the plant, and parked outside the entrance near the main road. Other employees saw him parked and contacted the owner, the owner contacted the employee's supervisor. The supervisor instructed one of the nearby drivers to stop and check on him. The driver called the employee and the employee said he was OK, which was relayed to the supervisor. The employee arrived at the the plant at about 10:15 am. The employee spoke to a coworker for a couple of minutes responsively but complained of a headache. The employee attempted to get out of the truck and had a seizure in the driver's seat and then went limp and was unresponsive. The coworker called 911 at 10:21 a.m. The temperature outside at that time was approximately 63 degrees Fahrenheit. The employee was taken by helicopter to a hospital and pronounced dead at 5:20 p.m. on April 27, 2015. The investigation concluded that the fatality was due to a non-work related medical condition.
