Abstract: At approximately 4:25 p.m. on September 1, 1992, Employee #1 was stapling a composition shingle roofing atop a 3-story house. His feet slipped on the shingle's granules, the small round stones on the composition shingle that came off as he walked on them. He slid to the roof edge, fell about 30 ft through the scaffold's railing below, and landed on the bed of a pickup truck. Suffering back strain and injury to two left ribs, Employee #1 required one week of hospitalization. No fall protection was provided. Employee #1 had not been given instructions or training regarding hazards and safety precautions applicable to roofing work. Employee #1 had about 17 years experience as a carpenter but very little experience as a roofer. The employer was found in violation of T8CCR 1730, and a serious citation was issued with penalty.
