Abstract: At approximately 11:30 a.m. on July 15, 1991, Employee #1 entered a 96 in. diameter reinforced concrete buried pipe to restart a gasoline engine water pump that was located 340 ft from the portal. The location of the pump qualifies it as a confined space. The pump had already run approximately two hours that morning from 8:30 a.m. to 10:30 a.m. As Employee #1 was attempting to exit, he collapsed and died 90 ft from the pump. His body was found and removed after 1:00 p.m. All documented evidence indicates that he went into the underground pipe to start the pump on his own volition, despite warnings not to do so and without employer knowledge.
