Abstract: On June 6, 1991, Employee #1 was operating the line 3 carton former. A friction roller on the roller conveyor of the line was squeaking. Employee #1 attempted to stop the roller manually while the line was running to find out which roller was making the noise. As he reached between the rollers, his right hand was pulled in and became caught in the belt-driven roller conveyor. Employee #1 was taken to a local hospital, where he was treated for bruises and lacerations to his right hand and released the same day. Employees are trained not to put any part of their hands in any moving roller conveyors. Any necessary maintenance/service should be reported to the maintenance department for response and service. Employee #1 received a corrective action interview as part of the employer's injury prevention program. Since the accident was not serious, the employer was not required to notify OSHA. The accident notification was made by the local fire department as the responding agency.
