Abstract: On December 14, 1994, Employee #1 was removing brake drums from a 10,300 pound fleet truck. His employer was going to troubleshoot the brake drums. Employee #1 used two Sears 12-ton hydraulic jacks, model #214.12630, purchased the evening before. The jacks were placed on either side of the rear of the vehicle to raise the rear of the truck off the ground. As Employee #1 was attempting to remove the left rear drum, the jacks fell to the side, allowing the vehicle to fall on his chest. Employee #1 died. Two 6-ton, Blue Point vehicle support stands, model #JS-006, from Snap-On Tools, also purchased the previous day, were found sitting on either side of the vehicle. They were not being used to support the truck.
