Abstract: A county road construction/repair crew was assigned to repair a section of gravel road by replacing a damaged culvert. Employee #1 was the crew foreman. As he stood within approximately 1 ft of the backhoe, which had dug out the shallow ditch and was now pulling a 4 ft by 2 ft section of concrete pipe over the damaged section of metal culvert, the chain attached to the pipe slipped. The arm of the backhoe jerked sharply and struck Employee #1. He did not lose consciousness, but later died of a damaged aorta during surgery. Employee #1 was hit because of the tension placed on the backhoe by the slight side pull angle.
