Abstract: At 11:00 p.m. on February 11, 1994, Employee #1 was operating a bulk saw at a veneer plant. He noted that the #6 log kicker kept cycling and thought an electric eye might have been blocked. He turned off the drag chains and, thinking he had turned off the kicker controls, went to inspect and possibly clean the eye. As Employee #1 walked down the drag chain to the #6 kicker bin, he apparently tripped the electric eye that caused the kicker arm to cycle. The bottom of the kicker arm swept across the conveyor, crushed his foot, and kicked him into the bin. He was transported to the hospital where he was treated for severe foot trauma.
