Abstract: At approximately 3:45 p.m. on November 24, 1995, Employee #1 was walking to various areas along a one-story 5:12 to 8:12 pitched roof on a residential structure, installing flashing around PVC pipe roof vents. He was accessing the peak of an 8:12 pitched section of the roof when he fell through a fireplace chimney opening cut out of the wood framing of the structure (i.e. a roof hole). Employee #1 fell at least 20 ft to the fireplace pit area and was killed. The roof hole had been covered with approximately 1 1/2 in. flat styrofoam sheeting.
