Abstract: At approximately 2:00 a.m. on July 19, 1995, Employee #1 and a coworker were attempting to clear a clogged 8 in. drain pipe located in a pit at the Copperweld Steel Co. They were using a specially-designed high pressure water hose and nozzle. The water was pressurized at 10,000 psi from a pump truck parked outside the building. The nozzle became stuck in the pipe and both employees attempted to pull the hose out while standing in front of the pit. The hose and nozzle broke free and started whipping around, striking and penetrating Employee #1's chest. He was killed.
