Abstract: At 11:30 a.m. on October 22, 2014, an employee completed adding chlorine to the water and was picking up chlorine bottles and placing them in a trash bag. The employee likely placed a hand on an energized part of the electrical service and was electrocuted. The electrical panels and outlets in the room were not in compliance with the National Electrical Codes. The Junction box was open and the capacitor box was open. There was no equipment grounding the conductor from the main breaker source to the subpanel. Existing grounding bar and neutral bar were not bonded together. The existing 240 volt feeding the well house pump was also operating a 120 volt outlet for a chlorination pump using grounding conductor of well pump as a neutral conductor for the chlorination pump. The surge capacitor was grounded to a metal water pipe instead of grounding bus. The surge capacitor was double lugged under the main breaker panel and needed it's own breaker. The original problem leading to many of the changes was the faulty mercury switch that had gone bad 2 years ago. It was used to turn the pump on and off when the tank filled but it had been bypassed and replaced with a toggle switch that was laying in the bottom of a box out of doors without a cover. Heat tape was fed by a series of household extension cords lying on the ground out of the door. It is not clear which one of the sources of energy the employee contacted. Based on the employee's height and the burns on his side and face it is likely contact was made with the open breaker panel or the capacitor box.
