Abstract: At 10:00 a.m. on October 7, 2014, an employee, plate welder, lost consciousness while grinding a tube joint inside the prefab building/tube shop. The employee was wearing personal protective equipment: Sundstrom SR 83 Welding Hood, Type C, Continuous-Flow Supplied-Air Respirator (NIOSH Approval TC-19C-428), welding gloves, ear plugs, BW GasAlert Clip Extreme 2 (BW Technologies Honeywell, Carbon Monoxide Single Gas Detector, GasAlert Clip, BW Model Number GA24XT-X), and steel-toe safety shoes. At the time of the incident, he was preparing a metal piece for Hull 21 for welding. The employee suffered a heart attack and died.
