Abstract: EMPLOYEE #1'S CLOTHES BECAME CAUGHT IN A PROTRUDING BELT. HIS LEFT THUMB THEN BECAME CAUGHT AND WAS AMPUTATED AT THE FIRST KNUCKLE. THE BELT DRIVE WAS NOT GUARDED.
