Abstract: At 11:30 a.m. on June 20, 2014, an employee was rigging a pallet of masonry block to be lifted via crane to where it was needed on the construction site. The crane utilized on site was a 90 Ton Hydraulic Grove Truck Crane (Year: 2004, Model: TMS900E, Serial Number: 223946). The rigging employee was working by himself out of view from the crane operator and any other employees on site. The rigging employee was in contact with the crane operator via a two-way radio. Once the rigging was set on the pallet of masonry block, the crane operator minimally lifted up on the load, at which point the crane boom sheared and collapsed to the ground below. The rigging employee was struck by the collapsing crane boom (specifically the jib extension) and received fatal crushing injuries to the skull.
