Abstract: At 5:00 p.m. on May 30, 2014, an employee was performing some light maintenance work on the tractor. The front driver wheel had been removed and an extended Big Red 20-ton jack resting on the dirt ground supported the front frame. On the driver side of the unit the four rear tires had been replaced by new ones and two extended jacks held the elevated tires in place clearing the dirt ground by approximately 1 inch. Immediately prior to the accident the employee was working underneath the truck frame directly under the cabin. The Big Red jack slipped off the front metal frame causing the driver side of the tractor to collapse to the ground, fatally injuring the employee. He was killed from blunt force trauma.
