Abstract: At 3:50 p.m. on May 29, 1991, as a result of a fire alarm, Employees #1 through #6 stopped their regular assignments and proceeded to the announced area to perform fire brigade duties. Duties involved controlling the fire in the material prep area at the #4 mixer prior to arrival of local fire department personnel. The product being manufactured had been discharged from the mixer into ductwork en route to another mixer. The fire in the ductwork produced thick smoke and extreme heat. All employees were treated for heat exhaustion and smoke inhalation at the local hospital; none required hospitalization. Their injuries resulted from inadequate fire brigade training to fight the fire and to select appropriate personal protective equipment for the potential hazards.
