Abstract: At about 9:15 a.m. on August 23, 1991, Employee #1 had just parked his tractor in the receiving lot of Ames, Inc., and was removing the third strap from a load of pipes when a 20 ft pipe, weighing about 244 lb, fell and landed on his chest, severing his aorta. The safety director of Ames, Inc., administered CPR to Employee #1, who was later transferred to Woodland Memorial Hospital. Employee #1 died. Employee #1's log book indicated that the pipe was loaded by the evening shift at Northwest Pipe and Casing Co. in Portland, OR, and that on August 21, 1991, Employee #1 left Portland at 9:00 a.m. On August 22, 1991 at 12:30 p.m., he checked the load, and again at the California state line at 4:00 p.m. He arrived in Woodland just after 9:00 a.m.
