At 7:30 a.m. on April 10, 2013, Employee #1 was accessing the upper levels of a three level storage rack to stock shelves. The employee was using a forklift, ex tended with a pallet on the forks. While standing on the elevated pallet, Employ ee #1 adjusted his position, and fell 10 feet to the concrete slab below. The em ployee sustained a fractured skull, killing him.
