Abstract: On October 23, 2011, Employee #1, a Maintenance Supervisor, was troubleshooting a small wheel abrader on second shift. Employee #1 took the guard off the abrader for the belt and pulley assembly to see what the problem was. His left hand came into contact with the unguarded belt/pulleys. Employee #1 then went back to the maintenance shop with his hand bleeding. Employee #1 told Employee #2, another maintenance man, he was hurt and Employee #2 took him to the hospital. Employee #1 arrived at the hospital on Wednesday evening, approximately 7:30 to 7:45 p.m. On October 19, 2011 to October 20, 2011, Employee #1 had additional surgery. Sometime Friday, he developed a blood clot in his left leg which resulted in no circulation. Employee #1 then had his left leg amputated below the knee. On Saturday, Employee #1 was having further problems, i.e., his kidneys were shutting down, and he was having blood pressure problems. He was then put on life support. On Sunday, October 23, 2011, at approximately 4:30 p.m., he was removed from life support. Employee #1 passed away at 5:00 p.m. Employee #1 did not lock out the equipment prior to troubleshooting the problem with the wheel abrader.
