Abstract: At approximately 8:00 a.m. on July 2, 1997, Employee #1, a carpenter, was working on the stage wall for a high school auditorium under construction. He was on a 10 ft stepladder when he fell 70 in. onto various length pieces of rebar that were protruding up from a concrete footing. An approximately 9 to 11 in. long piece of the rebar punctured his left temple, and a shorter piece went into his left calf muscle. Employee #1 was transported by ambulance to the Oktibbeha County General Hospital in Starkville, MS. Later that day, he was transferred by helicopter to the Northeast Mississippi Medical Center in Tupelo, where he remained in intensive care and on life-support equipment until his death at approximately 3:00 p.m. on July 5, 1997. According to the job superintendent, neither rebar caps nor wooden cover(s) were being used at the time of the accident. Preliminary information about this accident was provided to the Jackson, MS, area office at 9:08 a.m. on July 3, 1997, by the employer's office manager. The report of Employee #1's death was received by recorded telephone message at 6:30 p.m. on July 5, 1997.
