Abstract: At approximately 11:45 a.m. on February 24, 2012, an employee was preparing to use a portable Milwaukee 4-inch portable angle grinder with a coworker to remove a tack weld that had been placed on a socket. Employee #1 was using a Milwaukee handheld portable 4-inch wheel grinder that had been improperly equipped with a 7-inch cut off wheel. The wheel was too large for the device which required the removal of the guard so the wheel could fit. The grinder was owned by another company and the employee picked the grinder up from a shelf to use it as configured. When the employee applied power to the grinder, the wheel began to spin and failed causing the wheel to fragment and strike Employee #1's left index finger. Employee #1 sustained a fracture of the index finger requiring the insertion of a metal pin for stabilization as well as severe laceration requiring surgical intervention to save the finger.
