Abstract: At 4:00 p.m. on August 4, 2012 Employee #1 was headed to the fifth floor to take a break from servicing filters after complaining of dizziness. Employee #1 was working with two other employees. The estimated indoor ambient temperature was 104.5 degrees Fahrenheit. Employee #1 intended to use the facility installed Barnard and Leas continuous belt driven Manlift to descend the four floors to a break area on the fifth floor. When Employee #1 attempted to disembark the manlift on the fifth floor he failed to make the transition (faltered) and instead stayed on the manlift. After descending through the floor opening, he started shaking and dropped to a crouch on the manlift platform before finally letting go of the handhold. Employee #1 fell from the fourth floor to the basement. This is an estimated height of 60-70 feet. Severe laceration to the head, tramatic blood loss. Heat Stress was inconclusive as a causal factor. Employee #1 struck one or more objects while falling. Employee #1 was killed.
