Abstract: At approximately 4:30 p.m. on July 23, 2012, Employee #1, a mechanic, was carrying a fire extinguisher on the ground floor and walking towards a fixed ladder. He began climbing the fixed ladder to another level while carrying the fire extinguisher. A coworker had seen smoke in the conveyor system above the ground floor. Employee #1 and his supervisor investigated the smoke from an elevated platform and observed what appeared to be embers burning on one of the conveyors. Employee #1 left the platform to retrieve a water-based fire extinguisher and returned to the area with the extinguisher on a tricycle. The supervisor saw Employee #1 ascending the vertical fixed ladder with the fire extinguisher and then heard a loud thud and turned around to find Employee #1 laying on the ground. Employee #1 was unconscious with blood coming from his head and found approximately 6 ft from the ladder. Employee #1 died on July 27, 2012 from blunt force trauma to the head, including linear fractures of the right occipital and both temporal bones.
