Abstract: At 4:45 p.m. on July 12, 2012, an employee and a coworker were working in an elevated scissor lift. The employee was trying to lift and maneuver an 8 inch wide, 20 ft long channel steel beam into place. A portion of the steel beam had been supported by the backrest of a forklift truck operated by the president of the company. The employee and the coworker were elevated close to the ceiling. In the area where the employee and the coworker were working, was a pre-existing broken light fixture, which had exposed live electrical wires that ran to a suspended electrical metal conduit. The coworker in a separate scissor lift was also lifting and sliding the same 20 ft steel beam as directed by the employee. The employee was pulling the steel beam over the suspended electrical metal conduit, when his left hand came in contact with pre-existing exposed live wires that ran from a broken light fixture to the suspended electrical metal conduit. The employee came in contact with the exposed live wires and was electrocuted.
