Abstract: At approximately 8:15 a.m. on June 7, 2013, Employee #1, with World Kitchen LLC, was participating in an initial hands-on training session on how to operate a Crown Electric Pallet Rider (Model Number: PE4500-60; Serial Number: 6A301326). When Employee #1 attempted to stop, the pallet rider unit jerked. This caused Employee #1's lower left leg, above the ankle, to be caught between a wooden pallet, carried by an electric pallet jack, and the metal guard of a merchandise rack. Employee #1 was hospitalized for approximately three days for fractures to her lower left leg.
