Abstract: At approximately 5:20 p.m. on May 25, 2013, Employee #1, a security guard, was closing a manual horizontal sliding gate at a facility. She was primarily engaged in providing security by conducting security walk through of the property after all the employees left for the day. However, she also had the responsibility to open a southern gate at 4:30 a.m. and close this gate at 6:30 p.m. during weekdays and around the clock on weekends (starting for 6:30 p.m. on Friday). On May 25, 2013, Employee #1 was closing the southern horizontal sliding gate, which consisted of a single rolling section with dimension that were 5.92 ft tall and 43.2 ft wide and weighed 940 lbs. As she was trying to close the gate into its normal closing position, it went beyond its designated stopping point and fell on her causing traumatic and fatal bodily injuries. The cause of the accident was that the gate had not been equipped with positive stops or devices in order to limit the travel of the gate and not maintained in safe operating condition.
