Abstract: On January 16, 2014, Employee #1 a 63-year-old male with West Coast Pneumatics Inc., was using a portable 4 ft step ladder to access and troubleshoot a customer-owned compressor at their shop facility in Sacramento. Just before noon, as Employee #1 descended from the second to the first rung of the ladder, the ladder buckled, causing Employee #1 to lose his balance and fall from the ladder to the concrete shop floor. Emergency Medical Services were summoned and Employee #1 was transported to Sutter General Hospital in Sacramento, where he underwent surgery to repair a shattered femur in the left leg. Employee #1 was initially to be released after a stay of six nights in the hospital; however Employee #1 sustained a subsequent heart attack, while awaiting release, which required triple by-pass surgery, and a significant longer hospital stay.
