Abstract: At or about 2:30 PM on April 20, 2010, Employee #1, the operator of an electric walkie/rider (Yale, Model Number MPE060L), was picking product off the said racks and placing them on a pallet on the forks of his walkie rider. A coworker, the operator of a cushion ICE forklift (Yale, Model Number GLC070VX, 12,030 Lbs, 4,850 lbs capacity), was placing a pallet of 2-liter soft drink on top of another one located on the floor across the aisle from some racks. When the coworker was repositioning his forklift by going backward and turning, he ran over Employee #1's right foot injuring it seriously. Employee #1 was transported to Loma Linda University Medical Center, where he was admitted on April 20, 2010 at 7:15 p.m. and was discharged on April 21, 2010 at 1:14 p.m. The Cal/OSHA San Bernardino Office received the report of an industrial accident at 6:59 p.m. on April 20, 2010. Dr Pepper Snapple Group Inc., the employer, reported the accident to the Division in compliance with the reporting requirement of T8CCR, Section 342(a).
