Abstract: On January 21, 2014, Employee #1 and several other employees were loading portions of a rehearsal sound stage through a loading dock. They were using a forklift to place a dolly containing pipe racks for the stage. As the dolly was moved onto the dock, a space was created between the dolly and pipe rack. Employee #1 placed his finger into this space and the weight shifted on the dolly, crushing his right index finger and amputated his fingertip. He was taken to Cedars Sinai Emergency Room where they surgically reattached his fingertip. He was released the same day.
