Abstract: At approximately 7:30 a.m. on July 24, 2012, Employee #1, a painter was performing a sporadically assigned task of positioning the man lift to finish painting a building. The employee was positioning the man lift when he rolled over a utility cover. The man lift crushed the utility cover tipping the man lift over. Employee #1 was not wearing a harness and fell from the basket of the man lift. The employee suffered a fracture to his back in the fall. Employee #1 was transported to Huntington Memorial Hospital, where he was treated for a broke back and remains hospitalized.
