Abstract: At 10:28 am, on March 10th, 2010, Employee #1 of Dave Shaw Concrete & Block, Inc., died when he fell approximately fifteen feet into a bore hole excavation. Employee #1 was standing on a wooden platform placed on the fork attachment (Cat model #261-6681, serial number 65SSFP008870) of an Ingersoll-Rand F series Bobcat Loader (model number 863, serial number 514429551). Employee #1 was attempting to drive an 8 in. structural steel "I" beam into a 15 ft. deep by 2 ft. diameter hole inside a 4 ft. deep excavation using a Mikasa portable vibrating tamping rammer (model MT74F) when the accident occurred. The structural steel was intended to be part of a shoring system for this excavation. A pre-cast electrical vault (Oldcastle model Edison 61284VLT) was to be installed in this excavation. The tamping rammer dislodged the Caterpillar fork attachment and the platform where Employee #1 was standing. Employee #1 then held onto the tamping rammer and fell head first into the bore hole surrounding the piece of structural steel he was attempting to drive in with the tamping rammer. Employee #2, supervisor, had devised the pile driving procedure used at the time of the accident. Employee #2 was on another part of the worksite when the accident occurred. Employee #1 was engulfed in sandy soil and was asphyxiated. Employee #1 was pronounced dead at the scene by Los Angeles County fire paramedics.
