Abstract: On January 22, 2012, Employee #1 was unloading boxes of trash cans from a truck in the loading dock. He placed the trash cans on the pallet and shrink wrapped them, which were picked up by a forklift operator. Employee #1 started having back pain around 12:00 p.m., took some ibuprofen, and continued to work until 1:25 p.m. He then went to Pirate Staffing to pick up his paycheck and signed a document stating that he did not suffer work related injuries. On January 30, 2014, Employee #1 visited the Emergency Department at Citrus Valley Medical Center for back pain. He underwent emergency surgery for lumbar spinal cord compression and was discharged on February 1, 2014.
