Abstract: At approximately 10:00 a.m. on July 28, 2012, Employee #1, a 32-year-old male with Brown-Pacific Inc., was reducing the diameter of a wire on a one-inch draw block manufactured by Macbee, which was located in the production area. As the last of the material approached the die block, Employee #1 placed the open palm of his right hand near the coil of the wire wrapped around the draw block. Employee #1 was watching to see when the end of the wire would exit the die block so that he could apply pressure to the coil with his right hand preventing the end of the material from whipping around. As Employee #1 focused his attention on the die block, the glove on his right hand got caught between the loops of the wire coil and the block itself resulting in a complex laceration and sub-total amputation to the right index finger.
