Abstract: On July 15, 2010, an employee was driving a Toyota forklift in yard when radiator cap came off the radiator hose and hot water splashed on his back resulting in second degree burns to him. The employee was hospitalized one week in El Monte Community Hospital for medical treatment. The radiator water hose and associated cap on forklift was located very near to the driver's seat. It was also observed during inspection that radiator cap would not fit properly on the inlet hose and needed replacement.
