Abstract: At approximately 5: 50 a.m. on March 16, 2010, Employee #1 was using a portable Internal Chemical Tank Foamer, manufactured by Lafferty Equipment, with an attached high pressure hose, to conduct routine cleaning of meat processing machines. Employee #1 had filled the portable foam machine with 29 gallons of water and .75 gallon of Foam Klor S, to create a foam solution to spray on the food processing machines for cleaning. Employee #1 turned the pump on and felt a loss of pressure to the hose. He went back to the tank to check the pressure, then pinched and clamped the tip of the hose with his right hand while checking the top of the tank with the other hand. When full pressure resumed, the hose slipped out of Employee #1's hand, hit and dislodged his face shield, and struck him. The solution sprayed into Employee #1's left eye, which resulted in a corneal alkali burn. Paramedics arrived and Employee #1 was taken to Foothill Presbyterian Hospital in Glendora, California. Employee #1 was not wearing proper eye protection as prescribed by the Material Safety Data Sheet for the Foam Klor S.
