Abstract: At approximately 7:19 a.m. on March 28, 2008, Employee #1 was cleaning the inside of windows in a lobby of a two story building. He was using a 28 ft extension ladder and was approximately 14 to 16 ft high. He felt that the ladder was closing and jumped, landing on his side on the cement floor. Employee #1 was hospitalized for unknown injuries.
