Abstract: At approximately 7:00 a.m. on April 25, 2007, Employee #1 fell approximately 15 ft from the second floor of a building. A forklift driver lifted an unsecured wooden box containing tools to the second level of the building. The forklift driver was not certified to drive a forklift. Employee #1 was on the second floor to receive the tools but was not wearing fall protection. As Employee #1 stepped into the box to retrieve the tools, the box tipped over and fell causing Employee #1 to fall. Employee #1 was hospitalized with unspecified injuries.
