Abstract: On August 11, 2006, Employee #1 was operating a Crown order picker (Model Number SP3420-30, Serial Number 1A297696) to pick up a 47-in.wide, 48-in.-long, 8-in.-thick box that contained a wine rack (Sku Number 6004-350-1) located in a warehouse. He placed the merchandise on the right side of platform and safety gate of the lift and began to proceed backward heading eastbound. Employee #1's statement indicates his position was standing, facing forward, his right hand was on the throttle, left hand was free, his left foot was on the deadman switch, and he was looking over his left shoulder to make sure no one was walking or behind the aisle. He struck a box of furniture that was sticking out from a rack. Per the employer's accident investigation, Employee #1 may have placed the product upright on its side on the lift side of the order picker platform. That investigation indicated Employee #1's position was as follows: left hand on throttle, back of his body against the front view glass, and his right hand on the 27-in.-high left safety gate. The box of material crushed his right hand between the rack and safety gate. Employee #1 was hospitalized for compound fractures of his right index and middle fingers, and amputation of his right index finger.
