Abstract: At approximately 10:30 a.m. on July 15, 2012, Employee #1 and coworker #1, of Shin Shin Construction Corp., El Monte, CA, were unloading a ceramic counter top from a truck. Employee #1 was carrying the ceramic counter top with his left hand when he slipped on the sidewalk edge and fell to the ground. The ceramic counter top fell and landed on his left hand, crushing his middle and ring fingers. Coworker #1 informed coworker #2, the supervisor, who immediately called 911 emergency services. The Los Angeles County Fire Department responded and transported Employee #1 to Antelope Hospital, where he was diagnosed with an amputation to his third finger with bone loss. He was hospitalized for one day. This accident was reported to Cal/OSHA by the Los Angeles County Fire Department at approximately 7:12 p.m. on July 15, 2012. Cal/OSHA did not cite the employer for an accident violation. However, the employer, a company that performs interior remodeling construction work, was cited for other violations: T8CCR 342(a) Reporting of serious injury; T8CCR 1509(a) No training records; and T8CCR 1509(c) No code of safe practice on site.
