Abstract: At approximately 3:15 p.m. on April 6, 2012, Employee #1, a maintenance worker, was lubricating the chain of an adult tricycle. He used his right hand, which had a latex glove on it, to grease the chain and his left to pedal the bike. As he pedaled the bike, the tip of the glove of his pinky finger was caught and pulled into the moving chain and rotating sprocket. The pinch point of the chain and sprocket amputated the distal phalanx of the fifth digit on his right hand. Employee #1 was transported to a medical center. At the medical center, he underwent surgical procedures on the injured finger and was hospitalized for postoperative care.
