Abstract: On January 28, 2011, an employee was working as a sheet metal journeyman. He and another employee were assigned by the Foreman to remove some of the duct work in the cafeteria, which had to be rerouted. The two employees used two a-frame step ladders, one 10 feet and one 12 feet. The size of the duct work to be removed was approximately 30 inches wide by 18 inches high by 8 feet long. The two employees were working from the ladders, and they had removed the bolts from the point of connection at the existing duct work, releasing the straps and allowing one end of the duct work to be lowered down and rest on top of one ladder. When the bolts were being removed from the other end, apparently one of the screws was stripped and came out unexpectedly, releasing a section of the duct work. The duct work fell down and knocked one of the employees off their ladder. The employee fell approximately five feet and hit a countertop before landing on the concrete floor. The employee received four broken ribs and a bruised lung. The employee was treated and hospitalized at Providence Holy Cross Hospital.
