Abstract: On July 29, 2009, Employee #1 a sorter/mechanic at a recycling facility, was assisting a mechanic who was repairing the rollers on a three-mix glass sorting machine. The mechanic had been using a torch to smooth out the rollers on the sorting end of the conveyor line. He then directed Employee #1 to hold onto the v-belt on the motor for the conveyor and to pull on it to move the rollers while he watched the rollers to see if his work was complete. When the conveyor was actuated, Employee #1's right hand was caught in the moving belt and pulley. He was hospitalized for a partial amputation of his right index finger, a fractured right middle finger, and nail loss on his right ring finger. Employee #1 indicated that there was no warning that the conveyor would be started.
