Abstract: At approximately 2:00 p.m. on October 27, 2009, Employee #1, a foreman, and a coworker were working for an employer that specialized in structural steel fabrication. He was assisting in the construction of a restaurant, which required the steel fabrication company to install the ceiling of a cupola. They were working from an extension ladder, approximately 10 feet above ground level, to weld braces inside the cupola. As Employee #1 was carrying the extension ladder and walking on a 1.5 inch metal stud to exit the work area, he lost his balance and fell through the drywall to the concrete surface of the ground-level below. Employee #1 landed in a seated position, causing the severing of tendons in his lower back. Emergency services were contacted and Employee #1 was taken by ambulance to Henry Mayo Newhall Hospital, where he was hospitalized and treated for his injury.
