Abstract: At 6:45 p.m. on August 17, 2009, Employee #1 was assigned to clean the cutter, so that the broccoli could be processed. The machine used was a Kronen, Model GS-25 STD, Serial Number 228, electric 2-phase, 230-volt. The machine was not locked-out and taged-out, as required. Employee #1 placed his hand in the food processing cutter and amputated four fingers on his left hand. Employee #1 was transported to a local hospital for emergency treatment and was immediately transferred to Loma Linda Hospital in Riverside, California for reattachment of his fingers. Only one of his fingers could be reattached, and his middle, ring and pinky fingers on his left hand remained amputated. Employee #1 was hospitalized.
