Abstract: At approximately 6:00 p.m. on September 24, 2009, Employee #1 was installing a wood floor. Employee #1 was assigned to glue the wood floor pieces down and occasionally cut the pieces with the miter saw to fit at the walls. At the time of the accident, he was performing a straight cut and taking two inches off a 12 inch long piece of wood. The wood was about 1 inch thick and 3 inches wide. He had his right hand on the carriage handle and his left hand on the wood piece under the carriage. He slipped and his right hand pulled the carriage down while his left hand was still under the blade. Employee # 1 sustained injuries while using a 12 inch Sliding Compound Miter saw (Serial Number: 166105, Model Number: DW708). Employee #1 was transported for treatment to Cedars Sainai Medical Center. The company was primarily engaged in the installation of wood floors in residential sites. The accident occurred at a condominium complex on the balcony of one of the condominiums. Inspection of the miter saw involved in the accident found that it was in good mechanical shape with a properly functioning guard. There was no accident related violations found. Employee #1 suffered a lacerations to his long and ring finger and amputation of his small finger on his left hand. The Division was notified of the accident on September 24, 2009 at 8:46 pm, by the Los Angeles County Fire Department, and initiated an inspection on October 30, 2009.
