Abstract: On June 8, 2009, an employee and a coworker were at a jobsite and engaged in the installation of a commercial door. The employee was the lead for the job site, and the coworker was in training. The employee's supervisor was back at the office at the time of the incident. The employee stood on a six foot A-Frame ladder and was attempting to cut notches in the hinge behind the door frame with a Porter Cable portable grinder, Model Number PC750AG, attached with a four inch saw blade without a guard. As the employee was cutting the wood, the grinder kicked back and severely cut his left forearm. The employee was hospitalized and treated for lacerated finger. The employer did not ensure that the portable grinder with a four inch saw blade was appropriate for the tool and the task, and that the saw blade be guarded.
