Abstract: On January 21, 2009, Employee #1, a 63-year-old Project Engineer, was removing stored blueprints from the third tier of a three tier pallet rack. The blueprints were individually shelved and Employee #1 would remove single print sets and drop them to the table below. Employee #1 was working from the middle of an A-frame ladder and fell. Employee #1 suffering from an unspecified fracture, did not recall what height from which he fell but believed that he fell backwards away from the ladder in a direction parallel to the face of the pallet rack. The ladder was positioned such that the pallet rack was to Employee #1's left. There were no witnesses, and Employee #1 was working alone at the time of the incident. Statements by coworkers, who responded to the incident, told OSHA that the ladder Employee #1 was using remained upright after the fall. Examination of the ladder did not yield any violations and no citations were issued as a result of the investigation.
