Abstract: On May 2, 2006, Employee #1 was cutting a 2-in. by 4-in. board to be used for formwork for a footing. He sustained a partial amputation of his left index finger and was hospitalized for treatment. There were no witnesses to the accident. A supervisor was on site, but not in close proximity. The employer could not produce the hand saw that was used when the incident occurred. The exact cause of the accident was not determined.
