Abstract: At approximately 10:10 a.m. on September 9, 2006, Employee #1 was working for a contractor that specialized in the service and installation of metal ledgers and frames. A storage facility had contracted Employee # 1's employer to outfit their storage area with ledgers and frames. The project was estimated to be completed during a two-month period, starting in September of 2006. Employee #1 was instructed to drill holes on one of the walls at about 10 feet above the ground for the installation of a metal frame in one of the storage spaces on the second floor. While Employee #1 was on the second floor using an A-frame ladder, standing on a step that was about 4 feet above the ground, he fell and landed on his back. Employee #1 was hospitalized and treated for a fractured back. The accident investigation attributed the accident to Employee #1's lack of training.
