Abstract: On March 23, 2006, Employee #1 was on the roof dock of a Wal-Mart Super Warehouse working for contractor that was hired to install structural steel beams a metal roof deck. He was fastening the decking using a screw gun, when he inadvertently walked into an opening and fell approximately 19 ft landing on the dirt. He was taken to County USC Hospital by the paramedic, where he was later admitted for surgery. Employee #1 was hospitalized. The accident was caused by not providing personal fall protection to the employee before allowing him to work on the roof deck, where the potential fall distance was greater than 15 ft.
