Abstract: On December 30, 2005, Employee #1, a tree trimmer and grounds keeper, was using a portable powered hydraulic log splitter. The splitter had a Model Number TW5 with a Serial Number of B500240ACDDA01A and was manufactured by Timberwolf. The logs to be split were derived from tree trimming operations by the golf course employees. The logs were to be split, so as to provide firewood to club members. Employee #1 was loading logs onto the splitter from an adjacent pile, while his direct supervisor activated the controls. Employee #1's left index finger was amputated in the hydraulic log splitter, and he was hospitalized.
